The command to make disciples is clear, but how that's accomplished can be challenging.  Most pastors and leaders were never personally, intentionally discipled by another.  So how do they make disciples who make disciples?  Most churches have some form of discipleship-focused small groups, but how are those structured?  What training and equipping is provided?  What are the components of a health group?  What are your group philosophies and strategies?  How are leaders continually mentored?  What is your multiplication strategy?  How do you define discipleship and measure success?
